We recognize the variety of healthcare options available, especially when it comes to insurance coverage. With this in mind, below are our self pay/cash discount rates for a variety of exam services. Note that these are not limited to routine visits - they can be an option for you if you have a medical eye diagnosis as well. Also included is the CPT exam code.
Exams:
Comprehensive New Patient exam (Note: refraction fee is included for 1st time visits!) - $150 - CPT code 92004 & 92015
Comprehensive New Patient exam: Children (under 18, refraction fee is included for 1st time visits!) - $100 - CPT code 92004 & 92015
Comprehensive Established Patient exam - $120 - CPT code 92014
Intermediate (non dilated) New or Established patient exam - $100 - 92002 or 92012
Glasses and Contacts:
Refraction Fee - $40 - 92015
Note: Refraction is the procedure to measure your eye’s need for correction in order to see clearly and generate a prescription, which includes three steps: 1. Initial measurement of the refraction in both eyes, 2. Fine-tuning and subjective measurement of the prescription based on patient responses, 3. Creation of a finalized prescription, including any needed additional for reading (bifocal, trifocal, or progressive lens). This procedure is not covered by medical insurance and payment is due at time of service. Contact lenses require an additional visit for fitting due to their unique properties and risk of damage to the eye if not fitted properly.
Contact Lens Fitting Fee - New Patient - Standard Contact lens: $75; Specialty Contact lens (Toric/Multifocal): $125
Note: Due to their unique position directly on the eye, contact lenses are considered a medical device and require careful examination to ensure a good fit and good visual performance. This fee covers the following items: 1. Trial lenses which will be ordered for you, 2. Up to three fitting and exam visits to ensure proper fit and visual performance with your contacts, 3. Training and resources to help you learn how to safely and properly use contact lenses.

Contact lens Fitting Fee - Existing Patient - No change to Brand/Model, New Rx only - No charge
